Waiver & Release of Liability

Medical Release Waiver

I hereby give my permission for any supervisor, coach, or other team administrator associated with Old City Swim School ("OCSS") to seek and give appropriate medical attention to my child in the event of accident, injury, and/or illness. I will be responsible for any and all costs associated with any necessary medical attention and/or treatment. I hereby waive, release, and forever discharge and indemnify OCSS from all rights and claims for damages, injury, loss to person or property which may be sustained or occur during participation in OCSS activities, whether or not damages or loss is due to negligence. I hereby acknowledge that my child is physically fit and capable of participation in all OCSS activities.

Liability Waiver

By registering with OCSS, I agree to allow my child to participate in Old City Swim School programs, and hereby release OCSS, Cheviot Heights LLC, Gallaudet University, and their respective officers, directors, trustees, employees, coaches, volunteers, and agents from liability for any injury that might occur to my child while participating in OCSS programs, including travel to and from classes, lessons, training, meets, competitions, or other planned programming or activities. I agree to indemnify and hold harmless the above-mentioned organizations and/or individuals, their agents and/or employees, against any and all liability for personal injury, including injuries resulting in death to my child, or damage to my property, while participating in OCSS activities.

Media Release

I agree to grant Old City Swim School, its officers, directors, agents, coaches, and employees the right to record the image and/or voice of my child on video, film, photographs, digital media, and in any other form of electronic or print medium and to edit such recording at their discretion. I further grant OCSS the right to use my child's image and/or voice on the internet, in brochures, and in any other medium and hereby consent to such use. In the event I choose not to allow the use of my child's image and/or voice for said purpose, I agree that I must inform OCSS of this in writing at admin@oldcityswimschool.com or update my preference through the parent portal.

Assumption of Risk

I, the undersigned parent or legal guardian, acknowledge that swimming and water-related activities involve inherent risks, including but not limited to: drowning, near-drowning, injury, slipping on wet surfaces, exposure to waterborne illnesses, and other hazards associated with aquatic environments. I understand that no amount of instruction, supervision, or precaution can eliminate these risks entirely.

Pool Rules & Supervision

• I agree to ensure my child follows all pool rules and staff instructions during class.
• I understand that OCSS staff are responsible for my child only during scheduled class time, and that I am responsible for my child before and after class.
• I understand that OCSS reserves the right to remove any swimmer from class for safety reasons or behavioral concerns.

Confirmation of Understanding

I understand OCSS reserves the right to change these terms or terminate this program at any time. I understand notice may be given on or with my statement or by other methods. By checking the consent box during enrollment, I confirm that I have read and understood the above medical release waiver, liability waiver, media release, assumption of risk, and pool rules, and I indicate my agreement by completing the enrollment process. This waiver is binding on myself, my child, and our heirs, assigns, and representatives, and remains in effect for the duration of my child's enrollment with OCSS.
